This is the basic form to fill out.
The code is at the top and explanation of everything is below it, in case you don't understand anything.

Site Name: (What is the name of the site?)

Head Admin: (Who created the site, and if you did, don't just say me; we want a name)

Your Name: (Put the name of you on that site, not what you are on this site or any other site)

Site Link: (The link to the site so we know how to get there

Site Description: (Put at least 3 lines of description explaining what the site is about)

Banner Image: (If you have a banner image, put it here, if not, leave it blank)

Banner Code: (Again, if you dont have a banner, leave it blank. Please put the whole code, if you don't code it yourself, I'm not placing it up)

Other: (Is there anything you'd like to say to future members? And don't beg more members- it looks pathetic and no one will join. Are there any special features your site has that you feel no one else has? Post any extra material here.)
